4C Health REACH Grant Sees Early Success in First 90 Days
March 2025 – In just the first 90 days since launching in December 2024, the 4C Health REACH (Rural Enhancement and Access for Consumers Experiencing Homelessness) project has already made remarkable progress in reaching and assisting unhoused individuals across Miami, Cass, and Howard counties.
About the REACH Project
This five-year, grant-funded initiative is designed to:
Increase access to behavioral health and substance use treatment, harm reduction services, case management, and recovery support for individuals and families experiencing homelessness.
Expand community capacity to provide stable, permanent housing solutions.
Enhance access to whole health care and physician wellness services for those in need.
Strengthen stability in care and housing transitions through case management, care coordination, and service delivery planning.
A key component of this initiative is the Street Outreach Teams, which actively engage with unsheltered individuals, including those in encampments, as well as sheltered populations. These teams utilize specialized outreach and engagement strategies to improve access, participation, and retention in harm reduction programs, case management, treatment, and recovery support services.
Following the Housing First Model, the project prioritizes immediate and permanent housing solutions while also ensuring that individuals receive the essential support services needed to regain stability.
Early Success & Community Impact
To date, 4C Health’s outreach teams have exceeded year one goals and have connected with 130 individuals, conducting assessments and providing critical resources.
Alanna Lease, Director of Adult Intensive Services and Project Director of 4C Health, stated, “This project has opened my eyes in several ways to our community needs and the role that we all play. As we continue to navigate through resources, processes, and overall needs, the true demand for our program becomes more and more evident. I am so thankful to be apart of the tremendous work that we are doing and to assist our communities each day.”
4C Health’s dedicated homeless outreach teams are specially trained to address the unique needs of this population, ensuring that each person receives compassionate, effective care tailored to their situation.
